Failure Mode and Effects Analysis (FMEA) is a structured approach to discovering potential failures that may exist within the design of a manufacturing product or process.

Failure modes are the ways in which a process can fail. Effects are the ways that these failures can lead to waste, defects or harmful outcomes for the customer. Failure Mode and Effects Analysis is designed to identify, prioritize and limit these failure modes.

There are two broad categories of FMEA, Product FMEA and Process FMEA.

What is the need for FMEA?

  • The sooner a failure is discovered, the less it will cost. If a failure is discovered late in product development or launch, the impact is exponentially more devastating.

When to perform FMEA?

  • When you are designing a new product, process or service
  • When you are planning on performing an existing process in a different way
  • When you have a quality improvement goal for a specific
  • When you need to understand and improve the failures of a process

Features of MAPOL’s FMEA Application:

  • The system will be used to enter the ‘Machine’ and ‘Assembly’ process by using the same screens under different process name.
  • The System will auto calculate the ‘Occurrence’ value based on the inputs provided for ‘Actual’ and ‘Rejection’ quantities entered.
  • Provision to generate ‘Occurrence Ranking Matrix’ report from the FMEA screen.
  • Based on the changes in Process Flow’ screen, the further process will be changed in ‘FMEA’ screen by auto for that particular ‘Part’ number.
  • Based on the changes in FMEA screen, the further process will be changed in ‘Control Plan’ and ‘Work Standard’ screens by auto for that particular ‘Part’ number.
  • If the ‘Severity’ value exceeds 7, then system will auto trigger the alert emails to concern department/persons.
  • The System will trigger the ‘Email’ to concern person for ‘Approval’ by auto.
  • The escalation mail will also be triggered for the concern persons for escalation.
  • Provision to mark the changes as ‘Revision’ or not.
  • The complete ‘Log & Revision’ details will be captured in the System.
